Health Certificates for Jet-Setting Pets
At Kaua'i North Shore Animal Clinic we understand that traveling with your pet can be a confusing and complicated process. We are here to help you navigate the ins and outs of traveling both to and from to our beautiful island of Kaua’i.
We recommend every pet that is traveling by air or crossing state lines be accompanied by a Health Certificate (Certificate of Veterinary Inspection). Each state has different requirements for entry, our experienced team can help determine what your pet needs to make entry seamless. Health Certificates When You Need Them
When planning to travel with your pet,advance planning is key. We recommend you contact us to discuss your timelines and schedule your health certificate exam.
Get Prepared
In order to assist you with your pet’s Health Certificate we need information about your pet and your travel plans. You will need the following information available in order to fill out our Health Certificate Information Form.
- Origin and destination information
- Current and new owner information
- Carrier Information (if transported by a third party)
- Pet’s microchip and rabies vaccine information
This form will need to be submitted in advance of your appointment.
